چکیده

Electrograms from the bundle of His were recorded in two patients with acute myocardial infarction during paroxysmal atrioventricular block induced by carotid sinus massage. From the viewpoint of the surface electrocardiogram this arrhythmia was considered as a form of Type II (Mobitz) Ulo:k, the onset of which was related to a decrease in atrial rate. In the first patient, the conduction disturbance appeared at the AH, and in the second patient at the HV, level. Paroxysmal atrioventricular block could be explained by assuming the existence of: (a) vagal effects at the atrioventricular node or upper His bundle (Case I) and at 'lower' His bundle (Case 2); or (b) phase 4 block related to diastolic depolarization in 'upper' (Case i) and 'lower' (Case 2) parts of the His bundle; or (c) vagal effects at the atrioventricular node (Case I) and phase 4 block due to diastolic depolarization within the bundle of His (Case 2).

Complete His-Purkinje block produced by carotid sinus massage. Report of a case.

was evaluated for recurrent syncope. Right or left CSM produced brief periods of CHB with presyncopal symptoms. His bundle (HB) studies during normal sinus rhythm revealed normal conduction times (A-H interval = 80 msec; H-V interval = 48 msec). Carotid sinus massage produced progressive slowing of the sinus rate, and complete heart block below the HB occurred whenever the sinus rate fell below...

Paroxysmal atrioventricular block precipitated by an atrial premature beat. What is the mechanism?

A 65 year-old woman presented with two episodes of syncope. She developed paroxysmal atrioventricular (AV) block when an ECG was being recorded (Fig. 1). She developed at least one other similar episode. Then, she developed complete AV block with a stable escape ventricular rhythm of about 30 bpm during which an atrial premature beat was conducted (Fig. 2). Isoproterenol was administered which ...

Concealed paroxysmal atrioventricular block: diffuse congenital atrioventricular conduction system disorder with nonpropagated His bundle depolarizations.

A 2 1/2-year-old girl with bradycardia and left bundle branch block at birth began to experience "night cries" when deeply asleep. Electrophysiological study demonstrated congenital diffuse atrioventricular conduction disease with concealed paroxysmal atrioventricular block, nonpropagated His bundle depolarizations, severe sinus node abnormality, and a low atrioventricular junctional escape rhy...

His Bundle Electrocardiography during Bidirectional Tachycardia By STEPHEN N . MORRIS , M . D . , AND DOUGLAS

His bundle electrocardiography in a patient with a digitalis-induced bidirectional tachycardia revealed absence of His spike preceding earliest onset of ventricular activation during the bidirectional tachycardia. However, His potential preceded ventricular activation of normal complexes at a constant H-V interval of 35 msec. The presence of fusion QRS complexes, capture QRS complexes, the anal...
